In accepting an offer of a place in the Discipleship Training School (Certificate III in Discipleship 10473NAT) scheduled to commence star

I am fully aware of the contents, structure and duration of the course that I have contracted to undertake (as per Letter of Offer)

I understand that any claim for recognition of prior learning and/or credit transfer is subject to school policy and procedures and that the school’s decision will be final and binding

I have read and understood the ‘Student Handbook’ and ‘Course Information’

I am aware that as an international student I must fund my entire studies and living expenses independently and will leave Australia when my studies are complete and/or my visa expires.

That the basis of my enrolment at the school is on a full-time, full-fee paying basis only. I understand that it is neither a scholarship, employment or immigration program.

I am required to attend a two-day orientation program at the commencement of my course.

I confirm that I have read and understand payment schedule and refund policy (as stated below) and that all fees must be paid on or prior to my arrival, unless otherwise arranged beforehand. I also confirm that I am fully aware of my financial obligations, both to the Lord and to the students and staff at the school. I therefore accept all responsibility for my fees, tuition and personal expenses incurred during my involvement with Youth With A Mission.

I am aware of the cost of tuition fees & full board for the classroom phase, approximate costs for field assignment, living expenses and the right of the school to amend the costs. The costs of the field assignment phase may vary according to location. I am aware that the Registrar cannot accept any funds from me until I submit this signed agreement to verify my enrolment.

I understand that there is no reduction in fees should I receive an exemption for part of the course or should arrive after the scheduled course commencement date.

Youth With A Mission is only able to issue a transcript or Statement of Attainment for partial completion of a course which I have fully paid for, even if I pull out early.

Release of Liability- I do hereby release Youth With A Mission Inc, its agents, employees and volunteer assistants from any liability whatsoever arising out of any injury, damage or loss which may be sustained by said person during the course of involvement with Youth With A Mission.

Consent for Treatment- I hereby agree to the performance of such treatment, anaesthetics and operations as in the opinion of the attending physician if deemed necessary.

Consent for Burial- I agree that, in the case of my death while with Youth With A Mission, Youth With A Mission may carry out the burial in the place of the deceased. If my family desires to have my body shipped home, my family will pay for it. I hereby absolve Youth With A Mission and all its staff and associates of any burial costs.

Working with Children- I acknowledge that Youth With A Mission is a youth organisation and I will conduct my behaviour in the best interest of the youth in their care. I acknowledge that on acceptance I will be accountable to school leaders/ministry leaders to report any incidents that I am aware of regarding the welfare of the youth in the care of Youth With A Mission. I have completed the Working With Children Check “Student/Volunteer Declaration” as a part of the application process.

Advertising- I will allow my name, photo, quotes or testimonies to be used by Youth With a Mission, Newcastle and Institute for the Nations, for the purposes of advertising their courses and news articles.

Information is collected on this form and during your enrolment in order to meet our obligations under our regulations for enrolment of overseas students; to ensure student compliance with the conditions of their visas and their obligations under Australian immigration laws generally. Information collected about you on this form and during your enrolment can be provided, in certain circumstances, to the Australian governing authorities and. Information collected on this form or during your enrolment can be disclosed without your consent where authorised or required by law.

We will also use this information for registration with the University of the Nations, YWAM’s international training arm, to provide you with credits towards this course.
